State of the Union DIB Survey Research Incentive Program Terms and Conditions:

  1. Organizer
    The prize draw is organized by ISI Enterprises, whose registered office is at 250 Exchange Pl E, Herndon, VA 20170.

  2. Eligibility

    • The prize draw is open to legal residents of the United States who are at least 18 years of age and who received the original survey invitation email.
    • Employees of ISI Enterprises, its affiliates, or anyone professionally connected to the prize draw are not eligible to participate.

  3. Entry Requirements

    • To enter the prize draw, participants must complete the survey by March 31, 2025, at 11:59 PM EST.
    • A "survey respondent" is defined as an individual who fully submits all required responses via the survey form linked in the original invitation email.
    • Only one entry per person is permitted. Duplicate entries will be disqualified.

  4. Prize Details

    • All eligible respondents will receive a $100 Amazon gift card, up to a maximum of 100 recipients. 
    • Prizes are non-transferable and cannot be exchanged for cash or other alternatives.
    • The $100 incentive is limited to a total of 100 recipients across all phases of this research study. Incentives will be awarded on a first-come, first-served basis until the total allocation has been met, regardless of which survey group the participant belongs to.

  5. Notification and Timing

    • Gift cards will be awarded to the first 100 eligible respondents who complete the survey.
    • Recipients will be notified via the email address provided in the survey by April 15, 2025.
    • If a selected participant cannot be contacted within 14 days of the notification email being sent, the prize may be forfeited, and an alternative participant may be contacted should more than 100 eligible respondents exist.

  6. Privacy and Data Handling

    • Participant data will be handled in accordance with ISI Enterprises’ Privacy Policy.
    • Data will not be shared with third parties and will only be used for the purpose of administering the survey and prize draw.

  7. Tax Obligations: Recipients of the prize are responsible for understanding and fulfilling any federal, state, or local tax obligations associated with accepting the prize.The fair market value (FMV) of each prize is $100, which may need to be reported as taxable income. ISI Enterprises does not provide tax advice. Recipients are encouraged to consult a tax professional to address any specific questions regarding their individual tax situations.

  8. Liability

    • ISI Enterprises accepts no responsibility for entries not received due to technical issues, network delays, or any other reason outside of its control.
    • ISI Enterprises is not responsible for any damage, loss, or injury resulting from participation in the prize draw or acceptance of a prize, except where such liability cannot be excluded by law.

  9. General Terms

    • By entering the prize draw, participants agree to these Terms and Conditions.
    • ISI Enterprises reserves the right to cancel, amend, or suspend the prize draw in the event of unforeseen circumstances.
